July 22, 2008 Board Meeting Agenda Item #47 Title APPROVAL OF THE CONTRACT BETWEEN CYESIS SCHOOL AND LISA BOOTH Description Lisa Booth, independent contractor, will provide bilingual case management/social work services to teen parents, their children and their families to assist them in accessing necessary medical/social services.  The period of this contract will cover July 25, 2008 through June 19, 2009.  Two monthly payments of $1,600 each are due on the 15th day and the last day of each month. Recommendation That the contract between Cyesis School and Lisa Booth be approved. Contact NELSON & CANTEES Financial Impact $35,200.00
